		Register your child by printing out this form and returning it to the parish office or dropping it in the offertory. Due to the different sessions, each family is REQUIRED to register their children this year. If you are interested in participating in our Home Faith Program, indicate HOME on the registration form and we contact you. Please select the session you are requesting. Depending on the enrollment and catechist availability will determine the day or days each grade will be offered. Teens interested in the Summer Boot Camp, indicate SUMMER for the session.
